This form should be used by departments to request the destruction of records not covered in the university general schedule or approved university unit-specific addenda.
The Transcript Evaluation Inquiry Form is intended for use by students who have inquiries about how their transcripts were evaluated.
This agreement informs student employees and volunteers about their responsibility to protect identifiable information from student education records they may handle while performing tasks on behalf of NCCU.
Verification of credentials is required for all faculty, including adjuncts and full-time fixed-term faculty, regardless of the instructional delivery mode. See REG - 10.06.1 - Credentialing Requirements for Faculty Teaching Appointments.
Use this form to request items being moved from one location to another and/or sent to Surplus Property. All requests must be signed by the department head, include a contact number, and be faxed to 919-530-7988.
