International Student and Exchange Visitor Departure Notice

Occasionally, and for a variety of reasons, students and scholars leave the University early or unexpectedly. Some of these reasons include early graduation, leave of absence, suspension, expulsion, or family emergencies. You are required to inform the Office of International Affairs if you plan to leave the University before the date on your immigration document (I-20 or DS-2019) and provide the reason for doing so. You can do this by completing this form and returning it to our office prior to your departure. This form should not be used for summer vacations or school breaks.

This form should be completed whenever there have been changes in an exchange visitor's contact details or address during their time in the U.S. Please fill in and submit the requested information.

Clery Non-campus Property Registration

Locations that students use regularly for university-sponsored activities are subject to Clery reporting requirements. If you or your organization is using a space/property outside of the main campus boundaries in which you have a written agreement to use, please provide the following information. A written agreement can be anything from a formal contract to an email exchange agreeing to use a space/property.
