Slip, Trip and Fall Hazard Inspection form

This document contains the Slip, Trip and Fall Hazard inspection form. Everyone is responsible for identifying and reporting slip, trip, and fall hazards. Walk-through inspections of buildings and their surroundings are recommended at least annually using this form. Safety committee members should work with building inspectors to complete and submit copies of the results to EHS and Facilities Operations using this form.
