After you receive an affiliation agreement from an agency or hospital, the department should review it to see if it contains any of the following provisions and mark the applicable response. After the department completes an initial review, the department contact should submit this checklist along with the affiliation agreement to the Office of Legal Affairs.

The NCCU Fact Book: 2019–2020 provides the institutional profile as of Fall 2019 and contains a wide variety of information on new students, fall enrollment, degrees awarded, financial aid, faculty and staff, and university finance.
