Non-standard Affiliation Agreement Checklist

After you receive an affiliation agreement from an agency or hospital, the department should review it to see if it contains any of the following provisions and mark the applicable response. After the department completes an initial review, the department contact should submit this checklist along with the affiliation agreement to the Office of Legal Affairs.
