The CSA Annual Disclosure Form is used by Campus Security Authorities to certify whether they have reported all criminal incidents, arrests or disciplinary actions for Clery Act crimes reported to them in their capacity as a CSA. Only designated Campus Security Authorities should complete this form. 

This form allows users to submit titles for the library to purchase books.

This form must be completed electronically when new student interns begin work in the NCCU ITS department. Please ensure that all fields are completed. Questions regarding this form or status of requests should be directed to ITS at 919-530-7676.

NCCU employees who travel outside of the United States are required to notify NCCU of their travel plans prior to departure. This ensures that those who choose to travel are aware of return-to-work requirements and plan accordingly.
