This form must be submitted internally by the pre-determined due date when requesting financial support using Title III funding.
When programmatic changes are anticipated for your Title III activity/program, this form should be submitted as soon as possible for a timely internal approval.
Expenditure reports should be submitted to the Title III Office by the 30th of each month.
This report must be submitted internally by the pre-determined due date for review/approval prior to submission to the US Department of Education.

This travel report must be submitted internally immediately after the faculty/staff member returns from traveling.
Golden Eagles should submit the Reunion 2021 questionnaire if they plan to return for Homecoming 2021. The form should be submitted no later than July 16, 2021, to ensure Alumni Relations has an accurate headcount to plan for the event. Please share this link with other classmates to help spread the word.
