Special Event Vending Agreement
The Special Event Vending Agreement Form should be used to establish official permission to vend on North Carolina Central University campus or during University-affiliated events.
Mail Center Request Form
This form should be used to send mail through the University Mail Center using departmental funds.
Travel Liability Waiver Form
This form waives the University from liability when a person travels for a University event or via a University-owned or University-operated vehicle.
Workers’ Compensation Supervisor Statement Form
The Supervisor’s Accident/Incident Investigation Report Form should be completed and signed by the supervisor and submitted to the workers’ compensation administrator within 24 hours of a workplace injury.
