The Office of International Affairs (OIA) Affidavit of Support for Free Room and Board form should be completed only by the person providing free room and board to a student applying for an I-20. 

The Office of International Affairs (OIA) Affidavit of Support form is to be completed as a declaration of financial support by the student’s sponsor. If you have more than one sponsor, have each sponsor complete a form. This form is valid for 6 months only for.the purpose of issuing an I-20.
