Job Hazard Assessment (JHA) Template

A job hazard assessment (JHA) is a method for identifying and evaluating hazards associated with tasks (steps) with a specific job or activity and eliminating or mitigating them prior to conducting work. A quality JHA can prevent work-related injuries or illnesses by eliminating or controlling identified hazards.

Slip, Trip and Fall Hazard Inspection form

This document contains the Slip, Trip and Fall Hazard inspection form. Everyone is responsible for identifying and reporting slip, trip, and fall hazards. Walk-through inspections of buildings and their surroundings are recommended at least annually using this form.

Equipment Safety Clearance Form

This form must be completed and placed on all laboratory equipment prior to removal from the lab for repair/maintenance or for surplus/disposal to verify that all hazards have been removed and the equipment has been decontaminated.
