Purpose: In order to meet the parking demands of our growing campus, a single, unified form has been created to accommodate parking requests for guest parking and special events. This form will help organize both the handling of the requests and the scheduling of the request so that parking is not overburdened at any particular point in time.
If you have witnessed any possible fraud, waste, or abuse on campus, please use this form to report your observations.
Exchange visitors are required to notify the Office of International Affairs by completing this form if a spouse and/or children will be leaving the US earlier than the exchange visitor’s program end date and have no intention of returning.
